This is the official application form to apply for service aboard Starbase 386. You must be at least 13 years of age to apply. Please fill out every field, then submit the application to apply to join this ship; you should be contacted within 7 days.

Notes:
-An Officer would have 4 years of Academy training (Medical would have 4 years at Academy + 4 years at Starfleet Medical unless already holding a medical degree prior to beginning Starfleet Academy courses)
-Enlisted would have Basic Training for 7-13 weeks, then 3 months to 2 years of job specialty training
- Warrant Officers would have Basic Training for 7-13 weeks, then 3 months to 2 years of job specialty training + 3-4 years in the field
- Cadets are still in Academy.
